  • Date, time, location, subject and description of event.
  • Mayor's role at the event.
  • Contact person(s) name, organization, address, phone number and/or e-mail address.
  • Subject of meeting and detailed background information regarding issue to be discussed.

Aside from speaking, is there anything else the Mayor is being asked to do? i.e. present awards, pose for photos, etc.
How long does the Mayor need to attend? Please specify the start time and length in hours.
Speech, panel discussion, etc.
Podium, head table, etc. Include seating arrangements for the Mayor.
Who will attend? How many are expected to attend?
Will you accept a representative in the event the Mayor is unable to attend?
