Request a City Photo ID
To begin the photo ID request process, please contact Human Resources at hr@cityofmadison.com. Human Resources staff will follow up with more information about the request process, timeline(s), and (for replacement IDs) any cost(s) where applicable.
Please note that for security purposes, employees cannot request their own IDs, and the request must come from a supervisor/manager, Department/Division Head, or authorized administrative person in the employee’s agency/department.
