    To file a complaint with our office, please fill out a complaint form.

    Please provide as much information as possible when completing this form. If you do not want to share your identity, we will still process your form. However, we may need to contact you to conduct a full investigation.

  2. Meet with the Independent Monitor

    After you file a complaint, we will contact you to schedule a time to meet with the Independent Monitor.

    At this meeting, we'll discuss your complaint form and any additional information we need to conduct an investigation.

    You do not need to attend this meeting alone. You may bring anyone you wish with you for support.

  3. Investigation

    We will contact you to let you know if we are opening a full investigation on your complaint.

    Our staff will schedule regular check-ins with you during the investigation.

  4. Mediation with Police (optional)

    If you are comfortable doing so, we can arrange a mediation meeting with the Police Department.

    You would not need to attend the mediation alone. The Independent Monitor and your own legal counsel may attend with you.

  5. Review the conclusion of the investigation

    After we complete the investigation, we will invite you to another meeting with the Independent Monitor.

    At this meeting, the Independent Monitor will detail their Final Report on your complaint. The purpose of this meeting is to ensure you are satisfied with the scope of the investigation.

    If needed, Steps 3 and 4 will be repeated after this step.

  6. Final Report

    Once the Final Report is completed, it will be presented to the Police Civilian Oversight Board. You will be invited to meet with the Board in closed session to discuss the report privately with the Board and your feedback for the Office of the Independent Monitor.
